I printed out the sheet onto 160gsm card & cut out all the pieces. You need to punch the holes for the laces before you start assembling. To make it easier I scored around the edge of the shoe before I folded over the tabs.

Now the next bit is the fiddly bit. You need to glue the tabs to the base of the shoe. I found this easier by glueing around the base piece of the shoe rather than glueing the individual tabs. I added some narrow ribbon as laces & tied into a bow.
