I printed the sheet twice onto paper and cut out the picture square. I then folded these according to the instructions. I mounted them on a 3" by 12" piece of card. Then I wrapped a ribbon around it and fixed it at the back.

I used the remaining pictures from the sheet to decorate the outside of the card.
