Printed this design onto white card, then cut out all the elements, following the instructions I made up the main design base, then I made up the design using 2mm sticky pads

I printed onto matte card then fastened together at the top using wet glue. I added the base section with wet glue then attached the decoupage using 2mm foam pads.
