Used an 8" white square card. Added a lilac 4mm brad in the middle of the record. Glued 6mm gold pearls on the opposite corners. Added extra 3D squares under the Happy Birthday text to create a raised effect and added 3mm gold pearls on the panel.

Attached the matching insert inside with d/s tape.
