I printed the sheet onto glossy photo paper and cut out the pieces. I took a 5" square white base card that had ripple edges and stuck the backing panel diagonaly across the spine. I then stuck the base layer to the front. As I needed a card of low profile to go through on an ordinary stamp I only used every other layer, sticking them in place with 1mm deep, double sided, foam tape.

The bright colours on this card are very cheerful and would be suitable for a Get Well card as well as for a Birthday.
