I printed the sheet onto smooth white cardstock - cut out the elements and mounted the decourpage with foam tape.

I then adhered the black "Happy Birthday" peel-off to the sentiment area and added some faux pearls to the ribbon section.

I then mounted the base topper onto a matching cardstock to create the card base - used a piece of vellum inside and added the silver twine to the side to hold it in place.
