I printed the design onto gloss photo paper and cut it out. I made a gatefold card from patterned cardstock and glued the design to one side of the opening. I added the decoupage piece with foam pads and a birthday tag.

On the inside I printed the reduced design onto plain paper and a birthday verse underneath. I added some lacy borders on the side.
