Print onto 200GSM cardstock or photopaper. Cut out all pieces and attach main card front to blank card with DST or Glue. Stick decoupage layers on using 3D Foam tape.

I printed out sheet onto matt photo-paper, cut out and attached base to a scalloped edge white square card, added decoupage with foam pads and finished with a silver bow to spine.
