I printed this out onto good quality matt photo paper, I then cut out the design. Using navy blue card stock I folded it in half and glued the base picture in the centre, I then added the layers using foam sticky pads to give a 3d effect.

I finished off with a Get well soon peel off which I had coloured navy blue to match the card.
