I printed off the decoupage sheet and cut out the base image using a paper trimmer and small scissors to cut out the decoupage layers. I affixed the base layer using double sided tape and the decoupage layers using foam squares.

Perfect for a birthday but if you used the "Just For You" tag it could also be used as a get well or sympathy card.
