Print out the box on smooth card - I use 220gsm. Cut around the outlines, score and fold the crease lines and glue where indicated. Tuck the glued tags inside and make sure they're stuck well by smoothing with your fingers.

If you use wet glue you can use a clothes peg or paper clip to hold in place until the glue is dry.
